This issue related to social anxiety in children can be a major source of concern for parents and a real obstacle to the child’s development.

What Is Selective Mutism in Children?

Selective mutism is an anxiety disorder characterized by a persistent inability to speak in certain social situations (for example, at school or with strangers), even though the person is able to speak normally in other situations, such as at home or with close friends and family.

People with selective mutism do not intentionally refuse to speak, but their silence is often a response to intense anxiety. This disorder is often linked to a form of social anxiety, in which the child or adult feels paralyzed by the fear of being evaluated or judged by others.

Selective Mutism in Children

Selective mutism is generally observed in children and can lead to significant difficulties in social and academic interactions.

This problem is often overlooked because it is not very noticeable. In fact, the child's behavior is not disruptive. However, these children are at risk of developing social, language, and academic delays if no one supports them or helps them overcome their social anxiety.

Furthermore, because this issue is relatively rare, teachers and parents are not always sufficiently informed to support the child and respond appropriately.

Selective mutism in children generally begins before the age of 5, and there are usually two peaks in the incidence of the condition—around age 3 and around age 6—often linked to the change in environment associated with starting school.

Common signs of selective mutism in children:

  • Excessive shyness, fear of being embarrassed in public, isolation, and social withdrawal,
  • Excessive attachment to parents
  • Compulsive behaviors
  • Temper tantrums and defiant behavior, especially at home.

What causes selective mutism?

The causes of selective mutism are complex and may overlap:

  • Anxiety, Social Phobia
  • Separation anxiety associated with a close, symbiotic relationship with one or both parents
  • A change in environment or language due to a move or immigration
  • The use of confrontational, cryptic, or «synpraxic» communication (nonverbal communication focused on gestures and body language) at home

If you think selective mutism may be related to trauma or a difficult social or family environment, it is important to consult with a child psychologist or social worker.

Some Typical Cases of Selective Mutism in Children

Selective mutism can manifest itself in different ways depending on the child.

Here are a few common examples:

  • Selective Mutism at School:
    The child is quiet at school but expressive with his family. He is able to speak freely with his loved ones but freezes up completely in class. He avoids eye contact with teachers, remains impassive when others speak to him, and may even seem completely absent.
  • The child who talks only to a few classmates
    The child has one or two friends with whom he or she talks, but remains silent around teachers or other students. He or she sometimes whispers quietly to a trusted friend but refuses to speak directly to adults.
  • The child who doesn't speak at all outside the home
    The child remains completely silent outside the home, whether at school, with extended family, or in public places. He or she may seem frozen, tense, and avoid social situations, and may sometimes cry when told to speak.
  • A child who speaks only under very specific circumstances
    The child will speak only under certain conditions (for example, alone with a parent in a closed room or in a specific setting), but as soon as something changes (another person is present, a new situation arises), the child freezes up and stops speaking.

Hypnosis as an Approach to Selective Mutism in Children

Therapeutic hypnosis helps bypass conscious resistance and work with the unconscious mind—where fears and self-doubt reside—to help reduce anxiety and build self-confidence.

Hypnosis is particularly effective with children because it draws on their imagination, play, and relaxation.
In addition, hypnosis respects the child’s own pace, gently helping them to gradually try out new behaviors.

The goals of hypnosis therapy for selective mutism in children:

1era The first step, of course, is to build a relationship of trust between the child and the hypnotherapist.
By clearly explaining what will happen and making sure the child agrees to seek medical advice.

Next, therapeutic hypnosis for selective mutism in children generally focuses on four main areas:

  • Reducing Anxiety and change how we perceive certain anxiety-inducing situations.
  • Building Self-Esteem and self-confidence in social situations
  • Encourage gradual communication, First in whispers, then out loud in situations that might seem anxiety-inducing at first glance.
  • Empowering Parents so that they can continue the work at home and support their child's progress.

Case Study: Felix, Age 9

Félix, 9, has had selective mutism since he was 4.
This is evident in the school setting—where he speaks neither to the teacher nor to the other students—and in his social interactions more generally: he generally communicates only at home or in very familiar settings, avoiding speaking in the presence of people he doesn’t know or in social situations outside the family context.
The week following the 1stera During class, Félix surprised his mother by speaking to her in front of other adults at school, and even ordered a dish for himself at the restaurant.

Some Techniques Used in Hypnosis to Treat Selective Mutism in Children

Every child is different, but here are a few techniques we could use if it seems appropriate:

  • Deep Relaxation to reduce overall anxiety and ease internal tension.
  • Safety Anchor, for example, a subtle gesture (such as pressing a finger against the palm of your hand) to trigger a "bubble of courage" and calm yourself before speaking.
  • Gradual Desensitization where he mentally practices communication scenarios of increasing complexity. For example, he is guided through visualization exercises in which he begins by talking to himself in his room, then to a friend, and then gradually to a larger group...
  • Making Social Interactions Less Stressful through visualizations in which the child sees themselves successfully interacting with others without stress or imagines speaking in class with confidence
  • Therapeutic metaphors: For example, by imagining yourself in a story where a shy character finds the strength to talk to other characters and lets their voice be heard, while experiencing the positive feelings associated with that (joy, pride, relief).
  • Role-Playing Games For example, using stuffed animals, puppets, and drawings to help the child gradually express themselves without feeling pressured.

What Happens During a Hypnosis Session for Children?

Before the therapeutic hypnosis session:
We usually have an initial conversation with at least one of the parents over the phone to make sure we're the right resource for them.

It is important for the child to be willing to participate in the process.

Even if your child doesn't necessarily understand what hypnosis is, you can show them a photo or video introducing the hypnotherapist on our team to spark their interest in meeting this person who tells stories that are a little bit magical. 🙂

Often, just the word “hypnosis” piques their curiosity, and they can’t wait!

The Hypnosis Session for Children:
At the beginning of the meeting, the three of us—the parent who is present and the child—talk together.
After that, the session itself usually takes place with just the child.
At the end of the session, we have a quick debrief with the parent, but if everything goes well, we don’t go into detail—the child likes to keep that to themselves and will talk about it later when they feel like it.
